Did you know?
By providing us with your mobile number we will not only be able to send you text reminders for your appointments but we will also be able to remind you:
- when you are due for a chronic disease annual review
- when we need to update your medical records with information such as BP or smoking/alcohol status
- when we have a message for you and cannot reach you by home phone
You can update your contact details online or inform one of our receptionists of any changes to your details.
If you do not have a mobile number on file we may contact you through your home number using automated messages, through a company called MJOG. These messages may ask you to confirm your ID with your year of birth and generally will be for review or vaccination reminders. These calls come from the number 01353 780018 and are not a scam.
Record sharing allows health and social care staff outside your GP surgery to have access to your health information, approx 98% of patients already have a summary care record (SCR) which shares medications and allergies. In order to provide access to more detailed information which may be useful to services such as emergency departments, then you must consent to SCR with additional information. Please read the leaflet below for more info.
SCR + additional info leaflet
Please be aware that for patients over 16 with capacity we are not able to discuss treatment or management of the patient with family members/guardians unless the patient is also present.
Consent from the patient will allow us to disclose information already on file such as test results, or discuss matters such as arranging appointments or hospital transport with their nominated representative.
For patients lacking capacity. In order to release information or to be able to discuss treatments with family members or representatives we must see evidence of an existing lasting power of attorney or an order under the court of protection.
My Care Record
Our practice is part of the My Care Record approach which is supporting the work of health and care organisations across the East of England as they respond to the coronavirus (COVID-19) pandemic. For example, we are working to make sure doctors, nurses and co-ordination staff at places you may receive advice or treatment can see important information from your GP record. This could include NHS 111 or treatment centres such as the London NHS Nightingale Hospital. This will make it easier and faster for them to deliver effective care.
For more information, including the Privacy Notice, please visit the My Care Record website.
(Your CARE RECORD is different from your summary care record which is more basic.)
